INTRODUCTION

The questions in the questionnaire range from multiple choice questions to those that require a bit more writing. You are encouraged to express your thoughts and feelings freely in responding to these questions. I fully understand that these are some very personal issues and I respect your privacy. The information you provide will be kept safe and I will not disclose your personal details to anyone without prior written consent from you. You can participate through the online form here instead if providing your e-mail address is a concern to you.

Everyone who participates will have their name in the credit list of the work to be made available in the website and eventually, in the exhibition catalogs. You decide which name you would like me to use to credit you: it could be your first name, middle name, last name, initials, your nick name, screen name, etc. - completely up to you. If you wish to be completely anonymous, that is fine too. Just let me know.
